Role Overview

This position involves providing personalized, high-quality nursing care in patients' homes, utilizing the nursing process to assess, plan, implement, and evaluate care.

Responsibilities

• Assess, monitor, and evaluate patients' physical and psychosocial conditions.
• Collaborate with physicians, caregivers, and supervisors to coordinate care.
• Accurately document assessments, interventions, and patient progress.
• Educate patients and families on treatments and care plans.
• Follow established nursing standards and protocols.
• Perform additional nursing duties as assigned.

Qualifications & Skills

• Recent RN or LVN licensure; full-time availability; open to weekends and travel within the service area.
• Motivated to gain bedside experience through home healthcare.
• Flexible, with a commitment to a minimum of one year in the program.
