LVN/LPN PM Shift

Garden City Health Care Center, a skilled nursing facility located in Modesto, CA, is currently seeking qualified and caring Licensed Vocational Nurses (LVNs) to join our nursing team. Responsibilities

• Administrative & Leadership: Supervise and direct Certified Nursing Assistants during assigned shifts; oversee daily operations and ensure adherence to care plans and facility policies; participate in staff evaluations, orientation, training, and quality assurance initiatives; communicate effectively with residents, families, physicians, and interdisciplinary teams; respond to incidents, complete documentation, and escalate concerns to Nurse Supervisor.
• Clinical Care & Documentation: Administer medications and treatments per physician orders; monitor and chart resident health status and care interventions; support admissions, discharges, and transfers with proper documentation; participate in care planning and discharge planning processes; maintain accurate records in compliance with state, federal, and facility requirements.
• Infection Control & Safety: Enforce hand hygiene, infection control, and safety procedures; participate in emergency preparedness and fire/disaster drills; maintain a clean, safe, and sanitary work environment; ensure proper use of PPE and disposal of medical waste.

Qualifications

• Current LVN license in good standing with the California Board of Nursing
• CPR certification (required)
• Experience in a skilled nursing or long-term care setting strongly preferred
• Strong documentation, leadership, and communication skills
• Knowledge of PCC (PointClickCare) preferred
• Ability to lift, push, and move up to 25 lbs and assist in emergency situations
• Must pass background screening and meet facility health requirements

Pay

Competitive pay based on experience. Salary range: $28-$35 BOE.
