LVN Clinical Intake Coordinator (Home Health)

Job Summary

We are looking for an LVN Clinical Intake Coordinator to serve as a critical connection point between patients, families, physicians, and care teams ensuring seamless transitions into home health services and timely coordination of care.

Responsibilities

• Manage and coordinate incoming referrals, calls, and inquiries from patients, families, physicians, and healthcare partners
• Verify insurance coverage and ensure accurate intake processing
• Coordinate home health referrals and ensure timely patient onboarding
• Track hospitalized patients and facilitate smooth transitions into agency care
• Communicate effectively with nursing, therapy, social work, and physician teams
• Assist with scheduling and staffing coordination based on program needs
• Serve as a working coordinator for intake processes and workflow efficiency

Experience/Qualifications

• Active LVN (Licensed Vocational Nurse) certification
• Minimum 1 year of healthcare experience
• High School Diploma or GED
• PREFERRED: Associate’s or Technical Degree (or equivalent experience)
• 3+ years of healthcare experience, ideally in home health, intake, or care coordination
