LTSS Service Coordinator (Johnson County, IA)

About the role

The LTSS Service Coordinator role is based in Johnson County, IA. This position involves field-based work, primarily operating in the field to travel to client sites or designated locations as needed, with some office attendance for meetings or training.

Responsibilities

• Conduct service coordination functions for a defined caseload of individuals in specialized programs.
• Collaborate with the person supported to facilitate the Person Centered Planning process, documenting preferences, needs, and self-identified goals.
• Interact with Medical Directors and participate in interdisciplinary care rounds to support the development of a fully integrated care plan.
• Engage the member's circle of support and manage their physical health (PH), behavioral health (BH), and LTSS needs, as required by applicable state law and contract.
• Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities, ensuring cost-effective and efficient utilization of long-term services and supports.
• Document short and long-term service and support goals in collaboration with the member’s chosen care team, which may include caregivers, family, natural supports, service providers, and physicians.
• Identify members that could benefit from alternative levels of service or other waiver programs.
• May serve as a mentor, subject matter expert, or preceptor for new staff, assisting in the formal training of associates and participating in process improvement initiatives.
• Submit utilization/authorization requests to utilization management with documentation supporting and aligning with the individual’s care plan.
• Report critical incidents to appropriate internal and external parties, such as state and county agencies (Adult Protective Services, Law Enforcement).
• Participate in appeal or fair hearings, member grievances, appeals, and state audits.

Requirements

• Bachelor's degree in a health care-related field preferred.
• Minimum of 2 years of experience working with a social work agency or any combination of education and experience providing an equivalent background.
• Travels to worksite and other locations as necessary.

Skills

• Uses tools and pre-defined identification processes to identify members with potential clinical health care needs.
• Applies motivational interviewing techniques for evaluations and coordination of member cases.
• Coordinates cases with clinical healthcare management and interdisciplinary teams to provide care coordination support.
• Manages non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities.
