Responsibilities
- Provides services to customers and their families/significant others, to help them adjust to illness, injury or hospitalization.
- Affords the health team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes.
- Responsible for facilitating timely, efficient, and appropriate discharge of customers and coordination with next level of care, including transmission of appropriate plan of care.
- Identifies needed community services and coordinates appropriate services for customers in need.
Requirements
- Licensed Social Worker in the State of Minnesota
- 2+ Years Hospital Social Working Experience
- Discharge Planning Experience
- All Patient Populations Experience
- Epic Experience
