About the role
The Social Worker - LSW plays a crucial role in providing medical social work services to patients and families in a healthcare setting. This position works closely with the interdisciplinary team to ensure comprehensive care plans and address the psychosocial needs of patients and families.
Responsibilities
Provides ongoing assessment of patients, ensuring accurate evaluations of their needs.
Develops and implements comprehensive care plans addressing the psychosocial needs of patients and families.
Makes home health visits as needed, assisting patients and families with coping strategies and resource access.
Links patients and families with necessary services and supports, including funeral planning.
Evaluates bereavement risk and provides support throughout the bereavement process.
Participates in interdisciplinary team meetings and collaborates with team members to address crisis situations and ongoing care needs.
Fulfills quality and compliance responsibilities by following professional standards, monitoring care plans, and updating care plans as necessary.
Maintains accurate and up-to-date patient records using electronic medical record systems and other tools.
Maintains knowledge of regulatory requirements and stays current with relevant policies and guidelines.
Works closely with community resources and referral sources to provide comprehensive care.
Assists in the orientation, in-service, and continuing education of home health care staff.
Qualifications
Bachelor’s degree in social work required.
Current unencumbered license for the state of Ohio to practice as a Licensed Social Worker (LSW) required.
One year of social work experience in a healthcare setting required.
Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet preferred.
Requirements
Must be able to read, write, speak, and understand the English language.
Must possess a valid driver's license and acceptable driving record.
Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
