Strategic Responsibilities
Advocacy & Clinical Excellence
As the Social Services Director, you serve as the primary navigator for the resident’s journey, ensuring their voice is the loudest in the room:
- Holistic Clinical Assessment: Lead the identification of psychosocial, mental, and emotional needs. Move beyond the "Social History" to create a deep, narrative understanding of each resident's life and goals.
- Interdisciplinary Synthesis (MDS): Direct the Social Services portion of the MDS process, ensuring that psychosocial data is accurately captured to drive holistic, person-centered care plans.
- Proactive Discharge Navigation: Orchestrate complex transitions of care. Serve as the master liaison between the facility and community resources, ensuring every resident has a "fail-safe" plan for their next chapter.
- Rights & Dignity Stewardship: Act as the facility’s lead advocate for resident autonomy. Educate staff and families on resident rights, dignity, and self-determination, serving as a bulwark against neglect or exploitation.
- Crisis Intervention & Counseling: Provide expert emotional support and therapeutic communication for residents and families navigating the complexities of long-term care, cognitive decline, or end-of-life transitions.
- Regulatory Compliance: Ensure all documentation meets or exceeds stringent state and federal standards, maintaining an "audit-ready" department at all times.
- Community Networking: Cultivate relationships with Madison-area hospitals, VAs, and home health agencies to streamline resident support systems.
Professional Qualifications
- Education: Bachelor’s Degree in Social Work (BSW) is required.
- Licensure: Must hold an active, unencumbered LSW (Licensed Social Worker) or CSW (Certified Social Worker) license in the State of Wisconsin.
- Experience: 2+ years in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) is highly preferred. Understanding of the Wisconsin healthcare landscape is essential.
- Technical Savvy: High proficiency in Electronic Health Records (EHR) and a solid grasp of MDS 3.0 requirements.
- Soft Skills: Exceptional emotional intelligence (EQ) and the ability to mediate complex family dynamics with grace and professionalism.
