LSW Social Services Assistant

Full-time, day shift position with full benefits. Pay is negotiable, starting at $25/hour and up, based on experience and licensure. Opportunities for advancement available.

About the role

We provide skilled nursing care to people recently discharged from the hospital, needing therapy, and to those requiring long-term care.

Responsibilities

• Conducts Psychosocial Assessments: Evaluates a new resident's emotional, social, and psychological history.
• Develops Care Plans: Collaborates with the healthcare team to create and maintain individualized care plans and updates the interdisciplinary team on resident progress and goals.
• Emotional Support: Provides counseling to help residents cope with grief, loss, depression, anxiety, and the transition to a nursing home.
• Advocacy: Protects and promotes resident rights, ensuring voices are heard and needs are met.
• Grievance Resolution: Mediates complaints and reports any suspected abuse or neglect.
• Discharge Planning: Coordinates safe discharge plans, connecting residents with community resources like home health care or hospice.
• Resource Connection: Assists with connecting residents and families to legal, financial, and insurance case management.
• End-of-Life Support: Guides residents and families through advanced directives.

Requirements

• Licensed to work within the State of Maryland as a LSW Licensed Social Worker.
• Able to pass background check, drug screen, professional references check, and physical evaluation.
• High level understanding of leadership, customer service, and clinical competency.
• Leadership experience in a long-term care setting.
• Prior Long Term Care Experience (required).
