About the role

As part of a strong clinical care team, the primary purpose of this position is to assist in planning and delivering the facility’s social service programs. You will ensure that the medically related emotional and social needs of residents are met and maintained on an individual basis. This position reports to the Social Services Director/Manager.

Responsibilities

  • Assist the Social Service Director/Consultant in planning, developing, organizing, implementing, evaluating, and directing social service programs.
  • Meet with administration, medical and nursing staff, and other related departments to coordinate social services.
  • Obtain resources from community social, health, and welfare agencies to meet resident needs.
  • Provide solutions for social and practical environmental problems, including financial assistance, discharge planning, and referrals to community agencies.
  • Participate in community planning related to facility services and resident/family needs.
  • Engage in discharge planning, development and implementation of social care plans, and resident assessments.
  • Complete administrative tasks such as forms and reports for the Director.
  • Involve residents and families in planning social service programs when possible.
  • Arrange appointments and transportation to other facilities as needed.
  • Provide information to residents/families about Medicare/Medicaid and other financial assistance programs.
  • Compile and present discharge plan information to appropriate committees.
  • Work with facility consultants and implement recommended changes.
  • Make routine visits to residents and provide necessary services.
  • Deliver written and oral reports/recommendations to the Director about social services operations.
  • Assist in educating the community about aging, resident rights, facility services, and related topics.
  • Orient residents to the long-term care environment and facilitate their adjustment.
  • Attend in-service education programs to meet facility requirements.
  • Follow Standard Precautions, Exposure Control Plan, Fire Drill, and Evacuation Procedures.
  • Maintain confidentiality of resident and facility records.
  • Protect residents from neglect, mistreatment, and abuse.
  • Comply with Resident’s Rights and facility policies and procedures.
  • Report suspected or known fraud related to false billings, cost reports, or kickbacks.

Requirements

  • Good assessment, listening, and counseling skills.
  • Organized and detail-oriented work performance.
  • Excellent documentation skills; ability to write clearly to communicate patient care needs.
  • Genuine caring for and interest in residents of a nursing facility.
