Collaborates with patients, families, providers, and payers to assess care needs, coordinate services, facilitate safe and timely discharges, ensure appropriate resource utilization, and support revenue integrity through accurate documentation and compliance with coverage and reimbursement requirements.

Requirements
- New Hampshire LSW or LICSW license
- Social work degree
- BLS certification
- 2 years of case management experience in an acute hospital setting
- Acute hospital experience within the past year with no more than a 90-day break
- Critical care experience
Assignment Details
Length of assignment: 13 weeks
Shift: 8:00 AM – 4:30 PM
Hours per week: Full-time
