About the role

We are seeking Band 7 Independent Prescribing Pharmacists with at least 2 years of prescribing experience to join our Primary Care Pharmacotherapy Teams and support Community Hospitals. The role involves performing Pharmacotherapy tasks as outlined in the GMS Contract, including medication and polypharmacy reviews, assisting with discharge prescribing, and contributing to prescribing cost savings in Primary Care and Community Hospitals. You will also provide clinical support to staff and patients.

This is an opportunity for Independent Prescribers to broaden their experience and is ideal for those with existing expertise in this area.

Responsibilities

• Conduct medication and polypharmacy reviews as per the GMS Contract.
• Support discharge prescribing in Primary Care and Community Hospitals.

Requirements

• Band 7 Independent Prescribing Pharmacist qualification.
• Minimum of 2 years of prescribing experience.
