LPN, Virtual Care Onsite

About the role

Provide virtual nursing care to patients, members, and families to promote health and prevent illness. Collaborate with clinicians and leaders to deliver high-quality care within scope of practice.

Responsibilities

• Monitor and manage electronic messages related to patient and provider requests, ensuring timely resolution.
• Initiate follow-up calls based on message content and escalate queries outside scope to RNs.
• Conduct patient education and counseling, and evaluate the effectiveness of health teachings.
• Document clinical information accurately and participate in quality improvement activities.
• Advocate for patients, maintain confidentiality, and adhere to compliance standards and organizational policies.
• Perform additional department-specific duties as assigned.

Requirements

• Graduate of an accredited nursing program.
• Hold a Practical Nurse License (Georgia).
• Complete a Customer Service Assessment.
