Role: LPN supporting virtual care through technology-enabled patient services. Responsibilities Provide remote patient monitoring and triage messages.Assist providers using digital care platforms.Conduct virtual patient check-ins and document in the EHR.Coordinate follow-up care and educate patients on technology tools.Collaborate with RNs, physicians, and IT staff to resolve issues and optimize workflows.
