Support technology-enabled patient care as part of a Virtual Care team.
Responsibilities
- Conduct remote patient monitoring
- Triage patient messages
- Assist providers using digital care platforms
- Perform virtual check-ins
- Document in the electronic health record (EHR)
- Coordinate follow-up care
- Educate patients on technology tools
Collaboration
- Work closely with RNs, physicians, and IT staff to resolve issues and enhance workflows
Qualifications & Skills
