Responsibilities
- Oversee the clinical care and daily operations of an assigned nursing unit
- Supervise nursing staff, nursing assistants, and unit support staff while providing leadership and guidance
- Monitor resident conditions, review assessments, and ensure care plans are updated and implemented appropriately
- Cook with physicians, nurse practitioners, physician assistants, residents, and families
- Review laboratory results, provider orders, incident reports, and changes in resident condition
- Participate in care plan meetings, QAPI activities, family meetings, and interdisciplinary team discussions
- Assist with admissions, discharges, transfers, and coordination of outside appointments
- Ensure unit supplies, documentation, and resident records are accurate and current
- Support staff education, orientation, infection prevention efforts, and quality improvement initiatives
- Address resident, family, and staff concerns while promoting a positive care environment
- Perform additional duties as assigned
Requirements
- A High School Diploma or equivalent required; Associate's Degree preferred
- A Valid NYS Licensed Practical Nurse (LPN) License required
- A Minimum 1 year of LPN experience in a long-term care setting required
- A 3-5 years of long-term care experience with supervisory experience preferred
- A Strong knowledge of resident care planning, clinical documentation, and long-term care regulations
- A Leadership experience with the ability to supervise, coach, and mentor staff
- An Excellent communication, organizational, and problem-solving skills
- The Ability to coordinate care across multiple disciplines and manage competing priorities
