LPN, Scranton Hospice Center (PT, Nights, every other weekend)

About the Role

The Licensed Practical Nurse (LPN) provides direct nursing care to patients and performs a variety of indirect patient care duties under the direction of the Clinical Director and/or Registered Nurse. This role identifies patient and family needs, coordinates care, and contributes to an individualized plan of care as part of the Hospice Interdisciplinary Team (IDT). The LPN utilizes monitoring, intervention, and teaching skills to maximize patient comfort and enhance quality of life for patients and their families.

The position prioritizes patient and family comfort and care, delivering personalized, sincere, and engaging service while ensuring satisfaction and addressing concerns promptly.

Responsibilities

• Provide direct nursing care to patients.
• Perform indirect patient care duties.
• Identify patient and family needs and coordinate care accordingly.
• Contribute to and follow an individualized plan of care as part of the Hospice Interdisciplinary Team (IDT).
• Utilize monitoring, intervention, and teaching skills to enhance patient comfort and quality of life.
• Seek and accept guidance from IDT members and community resources as needed.
• Maintain accountability for patient satisfaction throughout the episode of care.
• Demonstrate problem-solving techniques to ensure appropriate interventions and resolutions.
• Communicate effectively and courteously with patients, caregivers, colleagues, and supervisors.
• Maintain confidentiality of patient information.
• Travel to patient homes as necessary.

Requirements

• Current licensure as a Practical Nurse in the Commonwealth of Pennsylvania.
• CPR certification (must be obtained and maintained).
• Clearance by PA State Police criminal history check and PA Department of Public Welfare Child Abuse Central Register (Childline).
• Graduate of an accredited Licensed Practical Nurse Program.
• Minimum of two years of general nursing experience.
• Hospice experience preferred.
• Experience with pain and symptom management interventions preferred.
• Understanding of Hospice philosophy and principles of death/dying.
• Ability to work within an interdisciplinary setting.

Physical Demands

• Medium Work: Exerting 25 to 50 lbs. of force occasionally (10-33% of shift), 10-25 lbs. of force frequently (33-66% of the time), and up to 10 lbs. of force constantly.
• Bending, reaching, stooping, and lifting (occasionally in strained positions) throughout the shift.
• Ability to transport self throughout the facility.
• Must be able to travel to patient homes if necessary.

Working Conditions

• Works in a state-of-the-art hospice inpatient unit.
• May work in varying conditions in patient homes (where conditions may be less than desirable).

Accident or Health Hazards

• Occasional exposure to blood and body substances.
• Possible exposure to needle sticks.
