LPN, Scranton Hospice Center (PT, Evenings, every other wknd.)
GENERAL SUMMARY
The Licensed Practical Nurse (LPN) provides direct nursing care to patients under the direction of the Clinical Director and/or Registered Nurse. This position identifies patient/family needs, coordinates care, and contributes to the Hospice Interdisciplinary Team (IDT) to enhance patient comfort and quality of life. The LPN pledges to prioritize patient and family comfort, provide personalized service, and ensure complete satisfaction.
EDUCATION
Graduate of an accredited Licensed Practical Nurse Program required.
WORK EXPERIENCE
Minimum of two years general nursing experience. Hospice experience preferred. Experience with pain and symptom management interventions preferred. Requires understanding of Hospice philosophy, principles of death/dying.
SPECIAL EMPLOYMENT REQUIREMENTS
- Current licensure as a Practical Nurse in the Commonwealth of Pennsylvania.
- CPR certification must be obtained and maintained.
- Clearance by PA State Police criminal history check and PA Department of Public Welfare Child Abuse Central Register (Childline).
FUNCTIONS
Works independently but seeks and accepts guidance from other members of the IDT and community as needed.
PHYSICAL DEMANDS
- Medium Work: duties involve lifting, carrying, pushing, pulling, and moving objects.
- Bending, reaching, stooping, and lifting (occasionally in strained position) throughout the shift.
- Able to transport self throughout facility.
WORKING CONDITIONS
Works in a state-of-the-art hospice inpatient unit. May work in varying conditions in patient homes (less than ideal).
