Sapphire Center for Rehabilitation of Central Queens, a premier long-term skilled nursing facility located in Queens, New York, is seeking Licensed Practical Nurses (LPNs) who desire a meaningful career making a difference in residents' lives.

Responsibilities

• Deliver nursing care in structured and routine situations.
• Administer and record medications and treatments as assigned.
• Prepare for and assist nurses and others with diagnostic procedures and treatments.
• Reinforce, support, and provide health care instructions to patients and their families.
• Participate with team members in planning, implementing, and evaluating nursing care and patient activities.
• Assist in the admission, transfer, and discharge of patients.
• Observe and report patients’ vital signs, symptoms, and general condition to appropriate individuals.
• Record observations of patients’ responses to nursing care on patient charts.
• Maintain environmental conditions conducive to patient comfort and safety.
• Assist with unit management and/or assume charge of unit as assigned.
• Perform other nursing functions related to patient care and nursing service, as delegated by Nursing Service Administration.
• Guide and direct technicians and nurse’s aides in rendering patient care.
• Maintain clinical competence through participation in continuing education programs and assistance with activities of daily living.

Requirements

• Completion of an educational program and NYS licensure as an LPN.
• Prior experience in a long-term skilled nursing facility is a plus.

Schedule

• Full-time evenings, nights, and weekends.

Pay

Competitive salary with an additional $3.224 shift differential for evening or night shifts.
