Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
• Participate in the development and implementation of individualized care plans, tailoring care to meet the unique needs of each resident and patient
• Collaborate with other healthcare professionals to ensure seamless and effective care delivery
• Demonstrate empathy, compassion, and respect for patients and their families, always prioritizing their comfort and well-being
• Maintain accurate and detailed records of patient care and communicate effectively with healthcare teams and families

Requirements

• Current LPN licensure and CPR certification
• Experience, with a passion for delivering high-quality patient care
