The interdisciplinary team works with each individual, their family, and physician to develop an effective plan of care that addresses mind, body, and spirit while respecting the rights and dignity of each resident.

Responsibilities

LPN

• Set up, administer, monitor, and chart oral, intramuscular and other medications and treatments.
• Observe residents for changes in physical or emotional condition, chart observations, and notify the supervising Registered Nurse and families as needed.
• Take and chart vital signs, blood sugar, and respiration.
• Communicate with patients to ensure comfort, explain therapies, treatments, and medications, and respond to assistance requests.
• Answer questions from family members and provide emotional support.
• Record physician’s telephone medical orders.
• Chart nutritional intake, bowel movements, fluid intake/output, and other resident activities.
• Report needs or changes to other departments (maintenance, dietary, housekeeping, laundry, PT, OT, pharmacy, beautician).
• Assist with maintaining supplies and medication inventories.
• Maintain knowledge of nursing principles, health care, sanitation, hygiene, and medical treatment.
• Attend care conferences, in‑service training, and staff/facility meetings.
• Direct visitors and monitor the alarm system for wandering residents, providing redirection as needed.

RN

• Evaluate residents’ physical and mental status, monitor vital signs, and assess response to treatments and medications.
• Obtain and implement physician’s orders; supervise subordinate nursing staff, ensuring proper technique and use of equipment, supplies, and materials.
• Prepare written evaluations, conduct new‑employee orientation, and maintain complete, accurate resident charts.
• Administer medications via oral, intramuscular, subcutaneous, intradermal, rectal, and intravenous routes; administer intravenous fluids.
• Prepare and implement resident care plans.
• Communicate with residents to ensure comfort, explain therapies, treatments, and medications, and respond to assistance requests.
• Answer questions and address complaints from family members; provide emotional support.
• Accompany physicians during rounds.
• Attend and participate in staff meetings, in‑service training, and continuing education.
• Order medications, supplies, and materials.
• Follow County safety policies and perform all job tasks safely.

Requirements

• Wage is based on title (LPN or RN).
• LPN: Prior practical nursing experience, preferably in an institutional setting, or equivalent education/experience; excellent customer service skills; strong oral and written communication abilities; computer literacy.
• RN: One to two years of nursing experience, preferably in a long‑term care facility, or equivalent education/experience; ability to supervise staff and implement care plans.
