Seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled home health nursing services with a focus on patient safety and quality care.
Responsibilities
- Implement care plans
- Participate in patient care conferences
- Document in electronic medical records
- Identify patient needs and communicate promptly with the RN case manager
- Administer treatments accurately
- Attend mandatory meetings and training
Requirements
- Graduate of an accredited nursing program
- Valid state LPN license
- At least one year of acute care experience, home health preferred
- Current CPR certification
- Virginia Driver’s license, reliable transportation, proof of auto insurance
