About the role
This program offers new graduate LPN/LVNs hands-on training by working 1-on-1 with patients in home settings. You will gain skills in direct patient care, vent/trach/g-tube management, and critical thinking, supported by experienced nurses providing mentorship and real-time feedback.
Responsibilities
- Using the nursing process to plan, implement, and evaluate patient care
- Administering medications and treatments, monitoring effectiveness, and documenting progress
- Responding to changes in patient condition and reporting to supervisors or physicians
- Documenting collaboration and coordination of care
- Ensuring safety of environment and equipment, adhering to universal precautions
Qualifications
- Graduate of an accredited nursing program
- Active LPN/LVN license
- Current CPR certification
- PPD or chest X-ray if applicable
