Fairmount Homes is seeking a dedicated Licensed Practical Nurse (LPN) / Charge Nurse to join our Personal Care team. Responsibilities

• Administering medications and treatments according to physician orders and facility policies
• Monitoring resident health and responding appropriately to changes in condition
• Making resident rounds to ensure safety, comfort, and quality care
• Completing required clinical documentation, progress notes, and resident records
• Supervising and supporting Resident Assistants while fostering accountability and teamwork
• Communicating resident needs and updates to team members and leadership
• Assisting with resident assessments, admissions, transfers, discharges, and end-of-life care
• Receiving and processing physician orders in accordance with facility policies
• Maintaining infection prevention, safety, and regulatory compliance standards
• Completing incident and accident reports as required
• Participating in team meetings and quality improvement efforts
• Supporting residents and families with compassion, professionalism, and respect

Qualifications

• Current Pennsylvania Licensed Practical Nurse (LPN) License required
• Strong clinical judgment and decision-making skills
• Ability to lead and support a team in a positive and respectful manner
• Commitment to resident-centered care and service excellence
• Strong communication and organizational skills
• Ability to remain calm and professional during challenging situations

Pay

Pay Range: $32.20 – $34.80 per hour. Shift Differentials: +$1.25/hour Weekend Differential | +$1.00/hour Evening Differential | +$0.50/hour Night Differential. Sign-On Bonus: $1,000.

Schedule

Part-Time, 24 hours per week, Day Shift. Work every Monday & Thursday plus every other weekend.
