About the Role

The Licensed Practical Nurse (LPN) provides quality home health care services within the scope of practice defined by state laws, following the patient’s care plan and collaborating with the healthcare team. The role requires independent work, good communication skills, and flexibility to meet patient needs in diverse home environments.

Responsibilities

• Deliver skilled nursing care in patients’ homes
• Educate families on care maintenance and health practices
• Monitor, record, and report patient status and changes
• Adapt to varied home settings and patient demographics
• Instruct families and observe/document patient responses

Requirements

• Current, unrestricted LPN license in the practice state
• Valid driver’s license, vehicle insurance, and reliable transportation
• Ability to work flexible hours
