LPN - Home Health - Per Diem

About the Role

The Per Diem Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) provides quality patient care in patients' homes, performing skilled nursing tasks within legal scope and the patient’s care plan.

Responsibilities

• Deliver nursing services per state laws and care plan, without altering the plan.
• Educate patients' families on care and environmental maintenance.
• Monitor, document, and report patient responses and condition changes daily.
• Adapt to diverse home environments regardless of patient demographics.

Requirements

• Current, unrestricted LPN or LVN license in the practice state.
• Valid driver’s license, vehicle insurance, reliable transportation or access to public transit.

Preferred Qualifications

• CPR certification.
• Home care experience.
• Ability to work independently and flexibly.
• Strong communication and organizational skills.
