Role Overview: The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely or chronically ill patients in their homes. Key Responsibilities Deliver high-quality clinical services within state-defined scope of practice, adhering to the care plan and infection control protocols.Thoroughly document patient visits and clinical notes in electronic medical records and visit verification systems.Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members. Qualifications & Skills Current unrestricted LPN licensure and CPR certification.Valid driver’s license, vehicle insurance, or access to reliable transportation.Ability to serve patients regardless of background or circumstances.
