About The Position
Monitor patients remotely using patient monitoring. Oversee patient management collaborating with Care Coordination and Remote Patient Medical Assistances. Assist with transition coaches in reaching out to patients for the two-day business call and any follow up calls.
Overview Of Responsibilities
- This position must be able to work independently within the scope of practice.
- Maintain patient alerts and address patient needs through virtual visits and telephone contacts by conducting a virtual health assessment.
- Ensure patients have orders and consents for remote patient monitoring.
- Discuss consent with patients reviewing the potential financial obligation by the patient.
- Manage and collaborate with Care Coordinators for on boarding.
- Complete the onboarding process for those patients who are identified as being appropriate for home visits or meeting at the main campus.
- Ensure patients are utilizing remote patient monitoring and complete follow up on those who are not utilizing.
- Ensure all patients who are enrolled have unique parameters set for each patient for the vitals that are being monitored within the program.
- Collaborate with the ordering provider for those patients who have abnormal vitals and adjust parameters per the direction of the provider.
- Collaborate with the ordering provider for graduation of the program for the patient.
- Ensure patients are able to self-manage before patient is discharged from program.
- Complete a manual charge entry for billing purposes of remote patient monitoring; this could potentially be up to five differently manual charge entries.
- Ensure all documentation is current in Reid Health’s electronic health as well as in the remote patient monitoring system.
- Run daily remote patient monitoring reports for equipment utilization to help meet the departments quality indicator.
- Provide summary and communication to the Care Coordinator about patient’s progress on remote patient monitoring for those patients enrolled into care coordination.
- Complete the transitional care management 1-2 business day call for those RHPA offices that do not have a licensed staff member.
- Complete the appropriate documentation for the transitional care management call.
- Communicate with RHPA regarding the transitional care management call to ensure that the patient has a timely follow-up appointment post-discharge.
Education & Experience
- Education Required: Graduation from an accredited Licensed Practical Nursing Program.
- Experience Required: Critical thinking and time management.
- Experience Preferred: At least 2 years’ experience in an acute care setting.
Licensure and Certification
- Licensure: Licensed Practical Nurse - Ohio and Indiana License.
- Certification: BLS.
