Responsibilities
- Adheres to the LPN scope of practice.
- Administers prescribed medications and treatments, as ordered by provider/Standard of Care Protocol.
- Provides patient care in the ambulatory care setting according to established standards in collaboration with Providers, Registered Nurses and Medical Assistants as members of the care team.
- Provides primary support to the provider by proactively anticipating the provider needs, preparing the patient for the visit, maintaining efficient patient flow, and assisting with treatments and procedures.
- Participates on interdisciplinary team to meet patient care and clinic needs, working cooperatively with others to accomplish required work in a timely and efficient manner.
- Assists providers with procedures.
- Assists with patient education, within scope of practice by Standard of Care-Education Protocol.
- Assists with visit planning, within Health Maintenance Guidelines.
- Aid in management, in collaboration with Clinical Manager.
- Documents in electronic medical record within scope of practice.
- Aid in screening of patients, in collaboration with Care Manager.
- Aid Certified Medical Assistants with patient preparation/rooming patients, as directed by visit plan.
- Applies Compassionate Connected Care principles and incorporates into care interactions.
- Interacts effectively with customers, providing instructions and information, both written and verbal, as appropriate.
- Maintains established practice policies and procedures, objectives, quality program, safety, environment, and infection control standards.
- Sets help, communicates, and cooperates with Medical Provider and team appropriately.
- Mets assigned productivity targets.
- Performs other functions as required.
- High School Diploma or GED.
- Current Licensed Practical Nurse-NMBLS required.
