Job Summary

About the Role
The LPN provides quality patient-centered, evidenced-based care to residents. They utilize Relationship Based Care principles and perform duties such as initial data collection and implementing person-driven care plans.
Responsibilities
- Utilize principles of Relationship Based Care
- Perform duties including initial data collection on all residents
- Implement person-driven care plans as developed by RNs
Requirements
- Current state licensure as a Practical Nurse in good standing
- Ability to read, write, and communicate in English
- Clinical experience preferred
