Summary Of Duties And Responsibilities
Responsible for providing practical nursing care and telephonic assistance to Variety Care patients and families. This position supports staff in the Patient Access Center, working collaboratively with both licensed and non-licensed team members to ensure high-quality patient care.
Primary Duties And Responsibilities
- Collaborates with the Registered Nurse and providers to facilitate next steps for clinical management of the patient with urgent care, primary care, telehealth or emergency services.
- Identifies patient’s personal strengths to reinforce patient education and recruit participation in improving and proactively maintaining their own health.
- Communicates barriers and patient’s health needs to the appropriate provider when indicated.
- Accurately documents all patient interactions utilizing the electronic healthcare record (EHR).
- Follows HIPAA guidelines and VARIETY CARE policy and procedures as it relates to privacy.
- Arranges for referrals outside VARIETY CARE, as requested by the provider. Advises patient accordingly. Answers questions regarding the referral.
- Assists with medical emergencies as directed by provider.
- Supports Variety Care’s accreditation as a Patient Centered Medical Home and our commitment to provide care to all Variety patients that is Safe, Effective, Patient Centered, Timely, Efficient, and Equitable.
- Works with all staff to achieve the goals of the “Quintuple Aim” of healthcare reform—to improve the experience of care, improve health outcomes, decrease healthcare costs, advance health equity and improve workforce well-being.
- Assists in the training and mentoring of Open Access and Patient Access Center staff.
Qualifications
- Graduate of an accredited LPN program.
- Active Oklahoma LPN license.
- BLS-HCP Certification.
- Three 3 plus years of clinical experience in emergency, urgent care, or primary care settings.
- Demonstrates ability to obtain and record accurate information.
- Ability to use or willing to learn electronic medical records software.
- Must be able to use basic computer skills including data entry and typing.
- Demonstrates, to the satisfaction of the RN or Clinical Director, an ability to administer medications and injections, has knowledge of instrumentation used in lab point of care testing, as directed.
- Able to communicate with the patient population.
- Able to maintain confidentiality with information.
- Able to work in a multi-faceted, fast-paced environment.
Preferred Requirements, Special Skills Or Knowledge
- Previous clinic or primary practice experience.
- Bilingual English/Spanish, speak, and comprehend.
