LPN/ NURSE IN HOME CARE PRN

Job Description

We have an IMMEDIATE HIRING NEED for the following position(s): Home Health Nurse – LPN

Responsibilities

• Assess patients’ condition and initiate start of care face-to-face visits
• Perform evaluation tasks including vital signs and medication review
• Administer medication as prescribed by the patient’s physician
• Dress (or re-dress) wounds
• Provide patient education
• Cookordination with the interdisciplinary team regarding care plan progress
• Make recommendations for devices/tools/medications that might improve patient’s quality of life

Job Requirements

• Must be 18 years or older and have successfully completed a FL-approved LPN program from an industry-accredited institution
• Must have reliable transportation and proof of auto insurance
• Must successfully complete a Level II background check prior to offer and initiation of training
• Must complete a competency check and all orientation prior to initiating patient care
• Current FL State LPN license is required
