Home Care Nurse - RN - LPN

About the role

Client is a 19‑year‑old male with traumatic brain injury (TBI). He is wheelchair bound, has limited speech, is hemiplegic, and requires neurological and/or behavioral health experience.

Responsibilities

• Medication set up and oversight
• Provide thoughtful care to clients based on personalized Plan of Care
• Provide in‑home assessments
• Monitor and track changes in the client’s condition and needs, and keep the Director of Nursing and other appropriate personnel informed
• Collaborate with the client’s family on plan of care, health changes, and concerns
• Provide total personal care

Requirements

• At least 1 year of nursing experience with neurological and/or behavioral health
• At least 1 year of nursing experience in home health
• Current unrestricted RN or LPN license in Utah
• Current CPR certification
• Negative TB skin test or chest x‑ray
• Ability to pass a drug test and background check
• Licensed driver with valid automobile insurance
• Ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull
• Ability to lift up to 75 pounds

Schedule

• Monday – Thursday: 2:30 PM – 7:00 PM
• Friday: 12:00 PM – 7:00 PM
• Saturday – Sunday: 7:00 AM – 7:00 PM
