LPN Nurse

About the role

The MDS Nurse is responsible for accurately and promptly completing the Resident Assessment Instrument (RAI), including the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care planning documentation, ensuring compliance with federal and state regulations. They support individualized resident care by reflecting each resident's clinical status in assessments.

Responsibilities

• Ensure compliance with federal and state regulations
• Support individualized resident care by reflecting each resident's clinical status in assessments

Qualifications

• LPN license with at least one year of experience
• CPR certification required
• Excellent assessment skills
• Ability to function independently and demonstrate leadership abilities
• Knowledge of RAI guidelines, PDPM, ICD-10 coding, and regulatory standards
• Prior MDS/RAI experience preferred
