LPN Nights-Hospice Crisis Care

About the role

This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.

Responsibilities

• Follow the plan of care for our continuous care patients
• Provide treatments as per the plan of care
• Collaborate with interdisciplinary team to support patients and families
• Document patient services accurately and timely
• Provide patient and family/caregiver education and information
• Evaluate patient responses to treatments/medications

Qualifications

• Graduate of an accredited practical nurse or a vocational nursing program
• Three years of nursing experience, preferably in community health, hospice, or medical/surgical settings
• Currently licensed as an LPN/LVN in the practicing state
• Current CPR Certification
• Good verbal and written communication, and organization skills
• Valid driver’s license with an automobile that is insured in accordance with state requirements
• Understanding of the hospice concept and the needs of the terminally ill
