LPN Nights-Hospice Crisis Care

About the role

This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.

Responsibilities

• Follows the plan of care for our continuous care patients
• Provides treatments as per the plan of care
• Works collaboratively with the interdisciplinary team
• Provides patient and family/caregiver education and information
• Accurately documents patient services reflecting the plan of care
• Evaluates patient responses to treatments/medications

Qualifications

• Graduate of an accredited practical nurse or a vocational nursing program
• Three years of nursing experience
• Community health, hospice or medical/surgical experience preferred
• Currently licensed as an LPN/LVN in the practicing state
• Current CPR Certification
• Demonstrates good verbal and written communication, and organization skills
• Valid driver’s license with an automobile that is insured in accordance with state requirements
• Understands the hospice concept and the needs of the terminally ill
