About the role
This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.
Responsibilities
- Follows the plan of care for our continuous care patients
- Provides treatments as per the plan of care
- Works collaboratively with the interdisciplinary team
- Provides patient and family/caregiver education and information
- Accurately documents patient services reflecting the plan of care
- Evaluates patient responses to treatments/medications
Qualifications
- Graduate of an accredited practical nurse or a vocational nursing program
- Three years of nursing experience
- Community health, hospice or medical/surgical experience preferred
- Currently licensed as an LPN/LVN in the practicing state
- Current CPR Certification
- Demonstrates good verbal and written communication, and organization skills
- Valid driver’s license with an automobile that is insured in accordance with state requirements
- Understands the hospice concept and the needs of the terminally ill
