About the Role

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team of healthcare professionals at Forest Manor Healthcare Center, a leading provider of skilled nursing care and rehabilitation in Blairstown, New Jersey. As an LPN on our morning shift, you will play a vital role in providing high-quality patient care to our residents and make a meaningful difference in the lives of our patients and their families.

Responsibilities

• Administer medications and treatments as prescribed by physicians
• Monitor and record vital signs, including blood pressure, temperature, and pulse
• Develop and implement individualized care plans, in collaboration with healthcare teams
• Provide wound care and maintain proper wound documentation
• Assist with daily living activities, such as bathing, dressing, and grooming
• Participate in quality improvement initiatives and maintain accurate records
• Collaborate with other healthcare professionals to ensure comprehensive care
