LPN -Medication Services-Adult

About the Role

Wyandot BHN is seeking a dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Medication Clinic. The role provides professional nursing care, care management, and coordination services to adult clients, supporting their treatment and overall well-being.

Responsibilities

• Engage individuals and families as partners in services and develop positive, hopeful, and trusting relationships.
• Assist individuals and families in avoiding and resolving crisis situations.
• Provide education and support to clients, families, or other identified support persons.
• Coordinate care with other treatment providers, both internally and externally.
• Support the prescribing physician and/or nurse practitioners as needed.
• Assist with scheduling within the medication clinic.
• Maintain an inventory of medical and office supplies for the medication clinic in collaboration with the Nurse Manager and administrative assistant.
• Interact with pharmaceutical representatives to coordinate with other providers.
• Make referrals for other services within Wyandot Center and other medical/psychiatric facilities per the prescriber’s orders.
• Prioritize urgent needs in collaboration with staff.
• Return phone calls to the clinic to assess level of need and develop a plan for accessing care.
• Coordinate prescription assistance plans with the pharmacy.
• Administer medications to clients, including injections.
• Complete all documentation and reporting requirements thoroughly and timely.
• Demonstrate positive interactions with clients, agency members, and external community partners.
• Comply with all relevant policies and procedures.
• Perform other duties as assigned by the immediate supervisor or designee.

Requirements

• Must be licensed in Kansas by the Kansas Board of Nursing as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
• Familiarity with mental health theory, emerging promising practices, and Evidence-Based practices.
• Prior experience with mental health is preferred.
• Knowledge of medical and non-medical services delivery systems.
• Effective communication with clients, families, guardians, and other service providers.
• Ability to establish relationships with individuals and organizations within the social services community.
• Demonstrated strong interpersonal and communication skills.

Work Conditions

This position is based at the 47th Street location. Transportation between Wyandot BHN locations or various community sites may be required to meet client needs.
