LPN Med Care Manager

Role: LPN Medication Nurse responsible for providing licensed nursing support in a senior living environment. Key Duties: Administer medications and treatments as per physician orders, follow the Six Rights of Medication Administration, accurately document in MAR/TAR, monitor symptoms, report adverse reactions, conduct Service & Health Updates (SHUs), transcribe orders, observe and report physical, emotional, or cognitive changes, maintain medication carts, and ensure compliance with regulatory standards. Qualifications & Skills: Active LPN license in good standing, CPR and First Aid certification, strong assessment and documentation skills, experience in senior care or medication administration preferred. Responsibilities: Play a critical role in preventing medication errors, early detection of health changes, and safeguarding resident safety through clinical accuracy and adherence to standards.
