LPN Med Care Manager

Role Overview The LPN Medication Nurse provides licensed nursing support to ensure safe medication administration, health monitoring, and accurate clinical documentation in a senior living environment. Key Responsibilities Administer medications and treatments following physician orders and the Six Rights of Medication AdministrationAccurately document in MAR/TAR in real-time and transcribe physician orders for care plan updatesMonitor residents for symptoms, adverse reactions, and physical, emotional, or cognitive changesReport any medication refusals or adverse reactions promptlyMaintain medication carts, perform narcotic counts, and ensure compliance with regulatory standardsConduct Service & Health Updates (SHUs) and support resident safety through clinical precision Qualifications & Skills Active LPN license in good standingCPR and First Aid certificationsStrong assessment, observation, and documentation skillsExperience in senior care or medication management preferred This role is critical in preventing medication errors, early detection of health changes, and ensuring resident safety through meticulous care delivery and regulatory adherence.
