LPN Med Care Manager

Responsibilities

• Administering medications and treatments accurately per physician orders
• Following the Six Rights of Medication Administration
• Documenting in MAR/TAR in real-time and supporting care plan updates
• Monitoring resident symptoms and reporting adverse reactions or refusals promptly
• Conducting Service & Health Updates (SHUs) and transcribing physician orders
• Observing and reporting physical, emotional, or cognitive changes
• Maintaining medication carts, completing narcotic counts, and ensuring compliance
• Preventing medication errors and safeguarding resident safety through clinical precision and adherence to regulations

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• PREFERRED: Experience in senior care or medication administration

This role is vital for ensuring safe medication management, early health change detection, and resident safety in a senior living environment.
