About the role

Provide direct patient care under the supervision of a registered nurse, including administering medications, performing vital signs, and assisting with daily living activities.
Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
Maintain accurate and detailed records of patient care and progress, ensuring compliance with regulatory requirements and facility policies.

Responsibilities

• Administer medications as per doctor's orders and patient needs.
• Perform vital signs checks and document results accurately.
• Absorb and assist patients with daily living activities as needed.
• Work collaboratively with interdisciplinary teams to develop and implement care plans.
• Keep detailed records of patient care and progress.
• Ensure compliance with regulatory requirements and facility policies.

Requirements

• Current LPN licensure in the state of Pennsylvania.
• 0-2 years of experience in a long-term care or rehabilitation setting.
• Basic Life Support (BLS) certification.
• Ability to lift, transfer, and reposition patients as needed.
• Excellent communication and interpersonal skills.
