About the role

Join our dedicated team at Milton Nursing and Rehab Center in Milton, Pennsylvania! As a valued member of our team, we are seeking a compassionate and skilled LPN Licensed Practical Nurse to provide high-quality care to our residents.

Responsibilities

• Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living (ADLs)
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Maintain accurate and up-to-date resident records, including medical charts, medication records, and incident reports
• Communicate effectively with families, healthcare professionals, and other staff members
• Participate in quality improvement initiatives, including identifying and implementing best practices

Requirements

• Current LPN license in the state of Pennsylvania
• Minimum 0 years of experience (new graduates welcome to apply)
• Ability to lift 50 pounds and move residents with assistance
• Basic Life Support (BLS) certification
