About the role
We are seeking a dedicated and compassionate LPN Licensed Practical Nurse to join our interdisciplinary team at Spring Creek Healthcare Center.
Responsibilities
- Provide direct patient care, including administering medications, performing treatments, and conducting vital signs
- Assist with daily living activities, such as bathing, dressing, and ambulating patients
- Develop and implement individualized care plans, in collaboration with our interdisciplinary team
- Maintain accurate and detailed records of patient care and services
- Monitor patient progress, identify changes in condition, and report to nursing staff
- Provide emotional support and education to patients and their families
- Collaborate with other healthcare professionals to ensure comprehensive care
Requirements
- Current LPN licensure in the state of New Jersey
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- High school diploma or equivalent required
