Responsibilities
- Provide 1:1 care to make a lasting impact on patients and families
- Offer greater work/life balance with flexible scheduling options
- Less time on your feet compared to other settings

- Give hands-on care to patients nearing the end of life, following state nursing rules
- Assess the patient’s needs and follow the care plan
- Keep clear and timely records of the care provided
- Teach and support patients and their families about the illness and care plan
- Communicate with the hospice team (like aides, chaplains, social workers) to help coordinate care
- Manage pain and symptoms, checking how well treatments are working
- Take part in training and quality improvement activities to grow professionally and improve care

Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Three years nursing experience
- Community health/hospice or medical/surgical experience is preferred
- Currently licensed as an LPN/LVN in the State
- Comply with accepted professional standards and practice
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Possess and maintain current CPR certification
- Understand the philosophy of hospice concept and the needs of the terminally ill
- Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems
- Demonstrate good verbal and written communication, and organization skills
