About the role
We are seeking a dedicated LPN to join our team at Phoenix Home Health. You will be an integral part of our patient care team, collaborating with therapists and physicians to implement personalized care plans.
Responsibilities
- Affirms and modifies the plan of care as directed by the physician and client’s payer source
- Performs nursing care in accordance with the plan of treatment and individual plan of care
- Monitors and reports client reactions to treatment and any changes in condition to appropriate personnel and/or physician
- Trains clients or families in the plan of treatment
- Attends education programs, quality assessment activities, nurse's meetings, and other staff meetings as required
Requirements
- Missouri or Compact Registered Nursing or Licensed Practical Nursing License
- Strong organizational and self-management skills
- Strong and compassionate customer service skills
- Valid driver's license
