About the role

We are seeking a dedicated LPN to join our team at Phoenix Home Health. The ideal candidate will be able to provide essential care and collaborate with various healthcare professionals.

Responsibilities

• Affix nursing care in line with physician’s treatment plan and individual plan of care, as per client’s payer source authorization
• Monitor, document, and communicate patient reactions to treatment and any changes in their condition to relevant personnel and physicians
• Provide ongoing patient or family education regarding the treatment plan
• Engage in educational programs, quality assessment activities, and staff meetings as needed

Requirements

• Missouri or Compact Registered Nursing or Licensed Practical Nursing License
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver’s license
