LPN (Licensed Practical Nurse) Home Health
About the role
We are seeking a dedicated LPN to join our team at Phoenix Home Health. You will be an integral part of our patient care team, working Monday through Friday from 8am to 5pm with occasional weekend rotations.
Responsibilities
- Affirms and modifies the plan of care as directed by the physician and client’s payer source
- Assesses and documents client reactions to treatment and any changes in their condition
- Trains clients or their families on the plan of treatment
- Attends education programs, quality assessment activities, and staff meetings
Requirements
- Missouri or Compact Registered Nursing or Licensed Practical Nursing License
- Strong organizational and self-management skills
- Strong and compassionate customer service skills
- Valid driver's license

$50,000 - $80,000
Schedule
Monday through Friday, 8am to 5pm with every 4 to 6 weeks, weekend rotation
