About the role
We are seeking an experienced LPN License Practical Nurse to join our team of dedicated healthcare professionals.
Responsibilities
- Provide direct patient care to assigned residents, including medication administration, wound care, and vital sign monitoring
- Develop and implement individualized care plans in collaboration with the interdisciplinary care team
- Monitor and report changes in patient condition to the nurse manager or Charge Nurse
- Participate in quality improvement initiatives to improve patient outcomes and satisfaction
- Maintain accurate and thorough records of patient care and treatment
Requirements
- LPN license (Practical Nurse) in the State of Pennsylvania
- Minimum 0 years of experience (new graduates welcome to apply!)
- Ability to lift 50 pounds and stand for long periods
