LPN in Assisted Living

About the role

Prosper at Wickliffe is seeking a motivated Licensed Practical Nurse (LPN) to join our assisted living community. The LPN works with the interdisciplinary team, residents, and families to assess and determine needs within an Individual Service Plan (ISP) and coordinate service provision as identified in the ISP. This role involves collaborating with the Resident Services Director and Caregivers to provide optimum care for each resident, general supervision of staff, and performing various technical nursing skills as needed.

Responsibilities

• Assist staff in providing direct care to residents, serving as a role model and assessing caregivers' skills.
• Act as a liaison between staff and administration, staff and families, and staff and service providers (including physicians) to promote optimal resident care.
• Collaborate with the Resident Services Director to develop and coordinate community educational programs, including orientation and in-service training for all personnel.
• Direct, evaluate, and supervise all resident care alongside the Resident Services Director, initiating corrective action as necessary.
• Review newly admitted residents and implement the Individual Service Plan (ISP) with the interdepartmental team for each resident.
• Identify safety hazards and initiate corrective action as needed.
• Complete reports and assignments accurately, adhering to established time schedules.
• Perform disciplinary action for staff when required.
