LPN in Assisted Living

About the role

Prosper at Wickliffe is seeking a motivated Licensed Practical Nurse (LPN) to join our assisted living community. The LPN works with the interdisciplinary team, residents, and families to assess needs, develop Individual Service Plans (ISPs), and coordinate service provision. This role involves collaborating with the Resident Services Director and caregivers to ensure optimum care for each resident, supervising staff, and performing various technical nursing skills as needed.

Responsibilities

• Assist staff in providing direct care to residents, serving as a role model and assessing caregivers' skills.
• Act as a liaison between staff and administration, families, and service providers (including physicians) to promote optimal resident care.
• Collaborate with the Resident Services Director to develop and coordinate community educational programs, including orientation and in-service training for all personnel.
• Direct, evaluate, and supervise all resident care, initiating corrective action as necessary.
• Review newly admitted residents and implement Individual Service Plans (ISPs) with the interdepartmental team.
• Identify safety hazards and initiate corrective action.
• Complete reports and assignments accurately and adhere to established time schedules.
• Perform technical nursing skills as required during the course of duty.
